Point Roberts Circle of Care Community Survey

Thank you for participating in our survey. Your feedback is important. We welcome your responses and your anonymity is guaranteed.  If, on the other hand, you would like to provide us with your name and details, a space for doing so is provided at the end of the survey. 

We are hoping to gather information about the people and families who live in Point Roberts who may currently need or are likely to need, in the future, in-home care.  Additionally, we would like to collect information about individuals  who might like to a become certified Home Care Aid. 

We could not adequately serve our community without the help of our volunteers, and as the needs of the community increase (as we expect that they will) our need for volunteers will increase proportionally.  If you have an interest in volunteering, please indicate that either in the space provided at the end of the survey or by contacting us directly on 360-945-5222 or prcircleofcare@gmail.com.
