This survey collects information for the Hawai‘i Neurotrauma Registry for the Hawai‘i Department of Health, Developmental Disabilities Division.
The Hawai‘i Department of Health, Developmental Disabilities has contracted with the Pacific Basin Rehabilitation Research and Training Center (PBRRTC) of the University of Hawai‘i to collect this information.
Your participation in the registry is voluntary.
Persons of any age in Hawai‘i with spinal cord injuries, brain injuries or who have had stroke are welcome to participate.
Your information will help identify ways to improve services to people with neurotraumatic injuries. You can fill out the survey at your own pace.
Your responses will be kept confidential to the limits of the law. This survey will be online until March 20, 2015.
